About Us
Our aim at Horizons is to help
everyone achieve their maximum level of independence.
Horizons Rehabilitation Centre
opened in June 1997, with Her Royal Highness The Princess Royal making
it official on 6th November 1997. The Centre is recognised for offering
a unique rehabilitation service in Scotland and an example of successful
joint working between Aberdeen City Council and NHS Grampian.
The Centre was purpose built to
provide assessment and therapeutic rehabilitation for people with physical
disabilities, aged between 16 and 65. Often these individuals can have complex
needs which require input from a number of different therapists. The aim
of the Centre is to meet the needs of our patients as well as their family
and carers.
We offer time-limited rehabilitation
programmes. Treatment assists individuals to achieve their goals and gain
maximum independence within the community.
We cover the Grampian Health
Board Area, which includes Aberdeen City, Aberdeenshire and Moray. Patients
are either living in the community or about to be discharged from hospital.
Transport
is available at the discretion of the Centre Manager.
Referrals to the Centre are
accepted from professionals in Healthcare and Social Work (referral must
be agreed by the patient's GP).
